At HammondCare, we don’t just talk about “holistic care”, but provide a service to our clients in an environment where our philosophy of care strongly supports client choice, independence and dignity. Our community care services are growing rapidly and in this managerial role, for our EACH program, you will provide leadership to a dedicated team of community care workers and will help deliver an innovative, individualized model of care.
