What’s wrong with our assessments and care plans?

Most assessments and care plans we see are based on a medical model of deficits. Hence care plans focus on managing and maintaining health from a clinical perspective.

Overwhelmingly, consumers are seeking support and services that enable them to live the life they want to.  This means focusing on care plans that enable, equip and empower consumers to maximise their abilities and focus support where it’s needed.  The care plan needs to be sharply focused on supporting consumers to achieve their goals and preferences.

Care plans should be a record of a plan of action negotiated with the consumer, to meet their health and social needs.

It should set out who is doing what, when, and why (outlines aims, actions and responsibilities)  with consideration to the safety of the consumer and others.
