Health Check-Ins in Home Care

Overview
Non-clinical staff must ensure that their care practices include regular and routine monitoring of consumers’ health and well-being. They also need effective mechanisms for referral to and engagement with clinical care providers. Failure to do so can jeopardise consumers’ well-being, especially those with health issues and complex care needs.
This training will equip you, as non-clinical staff, with the skills and knowledge to enhance ongoing monitoring of consumers by incorporating health check-ins into your care practices. You will learn how to ensure that your team understands the significance of regular health check-ins and how to implement them effectively to meet current and future requirements.
