This workshop focuses on bridging the gap between policy and practice, empowering care teams to translate regulatory requirements into effective operational strategies. Attendees will leave equipped with the knowledge and skills to design and implement a Care Partner Team model that aligns seamlessly with the principles of the Support at Home Program. You will gain valuable insights and practical tools to effectively manage both the clinical and care partner components of service delivery and strategies to deliver high-quality, participant-focused care that meets organisational objectives.
