Overview
In this two-session course, you will understand the implications of the reforms on care management. More importantly, this course will equip you with the skills, knowledge and insights to undertake care management at a higher level. It will ensure your service is ready and prepared to deliver care management that meets the needs of older people, compliance requirements and organisational efficiency. You will gain the essential skills that will equip you as a care manager to conduct independent assessments of clients, with a holistic, goal-oriented, wellness and reablement focus.
This course has been aligned with units of competency for the National Skills set -Case Management Skills. It consists of theory and practical application of learning in the workplace. It includes pre-reading, case studies and extensive resources. The modules include assessment, care planning, goal setting, health literacy, case conferencing, documentation, and monitoring of consumer changes. The training includes competency testing via an interactive learning style. The training has been modified for the home care environment to ensure practical examples of how learning can be translated into practice.
