Aged care software platform Acredia has launched a new home care solution – an integrated platform for residential aged care providers that also deliver home care, so that service management can be done on a single platform.

With the integrated platform, Acredia said providers can deliver consistent, person-centred care across service settings while improving operational efficiency and maintaining compliance with the new Aged Care Act and strengthened quality standards.

The platform is purpose-built for dual providers facing complexities around client management, employee scheduling, compliance and funding oversight. It features:

  • automated and mobile-enabled workflows to drive efficiency and retention
  • integrated service planning, goal tracking, and real-time collaboration across care environments
  • flexible scheduling models for shift-based, booking-based and client-centric care
  • streamlined financial management from centralised funding, billing, and cashflow
  • real-time updates and collaboration via the Acredia Home Care mobile app
  • live tracking of service plans, funding streams and future needs.

The Home Care Solution platform supports a variety of funding models, including home care packages, Commonwealth Home Support Program and DEX Claiming, National Disability Insurance Scheme, private services and insurance, emergency relief services and block funded programs.
